I don't think you can reasonably include Harold Shipman (or indeed any other serial killers), as they were not, by any stretch, part of any "system".

Agreed, not directly but 'the systemtm' failed to stop him early on, due to many failures within the poorly designed and managed system.
Blair has more 'blood' on his hands, but he lucky enough to enjoy the well paid fruits of his failures.
